BEEF DECLARATION FORMS

Beef declaration forms are due December 1, 2009 for the 2010 fairs. You can find them on the link below or on the Fair Forms link on the right side of the page. The $10. participation fee and member enrollment is due with the form which can be mailed to the 4-H office at 12220 Fillmore St. Suite 122 West Olive, MI 49460. You can also fax your form and then mail the payment. Checks should be payable to Ottawa County 4-H. Clubs are asked to submit all payments in one check if possible.
